Forms
For employees retiring over the age of 65 and need to enroll in Medicare Part A & Part B, please complete the top section of the CMS-L564E form, print it out, and bring to the MPUSD Insurance Department for a benefit representative to complete. The form will be required to have in hand when applying form Medicare Part B.
